  • 摘要: 目的 :研究SimPson-级切除突面脑膜瘤术后复发的原因. 方法 :通过突面脑膜瘤全切术后复发患者的临床、增强MRI、头CT及病理结果和手术切除范围等因素来分析术后肿瘤复发的原因. 结果 :脑膜瘤细胞沿硬脑膜浸润的范围较CT和增强MRI显示的范围广,术中切除硬脑膜范围不够是术后肿瘤复发的根本原因. 结论 :在SimPson-级切除基础上,切除增强MRI显示的硬脑膜尾征+0.3cm或不具备硬脑膜尾征的肿瘤累及硬脑膜区域以外0.5cm的硬脑膜和静脉窦,才能有效地阻止肿瘤的复发.

     

    Abstract: To investigate the pathogenesis of the recurrence of meningiomas operated by Simpson Grade I resection. Methods :The clinical,Gadolinum-enhance MRI,CT scan,pathology and the resection area were utilized to analysis the pathgesis of the recurrence of brain convexity meningiomas after total resection. Results :The dural area by meningiothelioma cell infiltration is larger than CT scan and Gadolinumenhance MRI,and the basic reason of recurrence of meningiomas is the less resect area of dural matter. Conclusion :Based on the operation by Simpson Gr-ade I resection,it can prevent the recurrenece of tumor effectively by resect the dural"tail"on Gaolinumenhance MRI+0.3cm,or the dural and vein-sinus away from the infiltrated area +0.5cm.
