上皮细胞钙粘蛋白(E-cadherin) 在膀胱移行细胞癌中的表达及其意义

E-cadherin Expression in Transitional Cell Carcin oma of Urinary Bladder

  • 摘要: 目的 观察上皮细胞钙粘蛋白(E-cad)表达与膀胱移行细 胞癌(TCC)分级、分期等生物学行为的关系。方法 采用恒冷切片免疫组 织化学SP法,对28例TCC和10例正常膀胱粘膜新鲜冰冻标本进行E-cad表达的检测。结果 对照组10例正常膀胱粘膜E-cad均正常表达,实验组28例TCC,仅50%(14/ 28)正常表达,二组差异显著(P<0.01)。TCC不同分级、分期间E-cad表达有显著性差 异(P<0.05),G3比G1+G2表达低,浸润性TCC比浅表性TCC表达低。初发与复发、 单发与 多发TCC之间比较,E-cad表达均未见显著性差异(P>0.05)。本组未见E-cad表达完全 缺失的TCC。结论 正常膀胱粘膜组织E-cad保留表达,TCCE-cad表达降 低,E-cad表达与TCC分级、分期呈负相关;E-cad表达与TCC的数目,与肿瘤的初发或复发 无关。

     

    Abstract: Objective To investigate the relationship between E-c adherin(E-cad)expression and histopathological grade,tumor stage and the other biological behavior in transitional cell carcinoma of urinary bladder.Me thods Immunohistochemical streptavidin-peroxidase method(SP) was emplo ye d using a mouse monoclonal antibody for human E-cad(HECD-1)to stain serial sna p-frozen sections of 10 normal bladder and 28 TCC specimens. Results E-cad expression was normal in 50%(14/28) of TCCs versus in 100%(10/10)o f normal bladder specimens(P<0.01).Abnormal expression of E-cad was signif icantly correlated with grade and stage of TCC,26.3%(5/19) of grade Ⅰ and grad e Ⅱ TCCsshowed abnormal E-cad expression compared with 100%(9/9) of grade Ⅲ TCCs(P<0.001),abnormal expression was found in 87.5%(7/8) of invasiv e TCCs compared with 35%(7/20) of superficial TCC(P<0.05).The differences were not statistically significant betwe en initial tumor with recurrent tumor and solitary tumor with multifocal tumor ( P>0.05).All of 14 TCCswith abnormal E-cad expression displayed heterogen eous staining, completely negative TCC had not been observed. Conclusion  E-cad expression in normal bladder transitional epithelia was preserved whe reas TCC showed a reduction of E-cad expression.Decreased E-cad expression was associated with both increased grade and stage of TCC.E-cad expression didn't c orrelate with the number of TCC,no difference was found between initial TCC with recurrent TCC.
