鼻咽癌调强放疗中腮腺平均吸收剂量预测模型研究

Prediction Model of Parotid Mean Dose in Intensity Modulated Radiation Therapy on Nasopharyngeal Carcinoma

    目的 探讨鼻咽癌调强放疗时腮腺平均吸收剂量(Dmean)及其D50%的预测模型。
    方法 随机选取西南医科大学附属医院肿瘤科50例鼻咽癌调强放疗患者,采集初始放疗计划CT图像,分析腮腺体积与其Dmean和D50%之间的相关数学模型,在制作放疗计划前对腮腺的Dmean及其D50%进行预测。
    结果 鼻咽癌患者Voverlap/Vparotid与Dmean/Dprescription之间存在显著正相关性(置信度为0.01, r=0.895),Voverlap/Vparotid与D50%/Dprescription之间存在显著正相关性(置信度为0.01, r=0.812),并利用Matlab软件拟合出上述两者之间的相关公式。
    结论 鼻咽癌患者在制作放疗计划前可根据本研究所拟合出的相关数学模型,在给定处方条件下通过Voverlap/Vparotid,提前预测出Dmean与D50%,作为调强计划评估标准,从而减少放疗计划优化过程中主观因素造成的影响。

     

    Abstract:
    Objective To investigate the prediction model of parotid mean dose and D50% in intensity modulated radiation therapy (IMRT) on nasopharyngeal carcinoma (NPC).
    Methods We selected 50 NPC patients who underwent IMRT, scanned the radiotherapy plan CT, analyzed the relationship between the parotid volume and its mean absorbed dose and D50%, and predicted parotid mean dose and D50% before planning IMRT.
    Results There were significant correlation between Vparotid/Vparotid and Dmean/Dprescription (confidence coefficient=0.01, r=0.895), Voverlap/Vparotid and D50%/Dprescription (confidence coefficient=0.01, r=0.812). Matlab software was used to fit the correlation formula.
    Conclusion For the patients who underwent IMRT, we can predict Dmean and D50% by Voverlap/Vparotid under the prescription according to the relevant mathematical model fitted in this study before making the radiotherapy plan, and reduce the impact of subjective factors in the radiotherapy plan optimization process, as a standard for assessing programs.
