Treatment of Advanced Gastric Carcinoma by Gastric Artery Intervention Chemoembolization Combined with Radical Cure Resection

摘要

摘要: 目的探讨进展期胃癌介入化疗栓塞联合根治术的切除率和生存率。方法对63例进展期胃癌患者术前行胃动脉介入化疗栓塞后手术，并与术前常规静脉化疗后手术56例进展期胃癌对照组的切除率和随访结果比较。结果胃动脉介入化疗栓塞后对进展期胃癌及合并呕血、便血的效果显著，根治性切除率达100％，对照治疗组根治性切除率达55．36％，姑息性切除术28．57％，转流术16．07％。研究组1／2、1、2、3、4、5、6年生存率分别为100％（63／63），93．65％（59／63），93．22%（55／59），90．90％（50／55），58．00%（29／50），41．37%（12／29），25．00％（3／12）。对照组1／2、1、2、3、4、5、6生存率（无6年生存者）分别为83．93％（47／56），87．23%（41／47），70．73％（29／41），72．41％（21／29），42．86%（9／21），11．11％（1／9），0．00％。经统计学处理，两组1～3年生存率有差异（P〈0．05）。结论对进展期胃癌胃动脉多次介入化疗末次栓塞，可使肿瘤病灶缩小，有利于防止转移和复发，提高手术切除率和延长生存率。

Abstract: Objective 　Reseach the resection and survival rate of advanced gast ric carcinoma af ter the intervention chemoembolization and radical cure resection. Methods 　Compare the resection rate and patients survival bettwen 63 cases after intervention chemoembolization with 53 cases after general chemotherapy. Results 　The effect of intervention chemoembolization was remarkable on the symptom of hematemesis or hematochezia, the rate of radical cure resection reached 100 %, the cont rast group reached 55. 36 %, palliative resection reached 28. 57 %, transition 16. 07 %. The 1/ 2, 1, 2, 3, 4, 5, 6 year survival rate were 100 %, 93. 65 %, 93. 22 %, 90. 90 %, 58. 00 %, 41. 37 %, 25. 00 %. while the 1/ 2, 1, 2, 3, 4, 5, 6 year survival rate (no 6 year survivals ) for the cont rast group were 83. 93 %, 87. 23 %, 70. 73 %, 72. 41 %, 42. 86 %, 11. 11 %, 0. 00 %. The 1～3 year survival rate between two groups has differentia dealt with statistics ( P< 0. 05) . Conclusion 　Intervention chemotherapy times and embolize at the last time, can reduct the tumor focus, prevent t ransference and recurrence, improve the rate of resection and survival for advanced gastric carcinoma.
