  • 摘要: 日益革新的癌症预防和治疗策略在多种恶性肿瘤的防治中收效显著,而胰腺癌发病率和死亡率致死率的上升趋势仍未出现明显转机。中医证候物质基础的阐明可以为胰腺癌的多学科整合诊治添砖加瓦。长期临床实践和调研明确了湿热证是贯穿胰腺癌病程患者的核心证候,其重要物质基础组成为肿瘤炎性微环境,清热化湿方可以通过重塑肿瘤炎性微环境抑制胰腺癌恶性表型。临床中,清热化湿治疗可以提高联合清热化湿的综合治疗方案可以使胰腺癌患者的生存质量得到改善,延长生存时间显著延长。通过建立标准化湿热证动物模型进行基础研究,发现炎性微环境是湿热证的重要物质基础组成。临床转化研究表明系统性炎症反应指数(systemic inflammatory response index, SIRI)可以与湿热型胰腺癌细胞因子的改变密切相关,可以指导胰腺癌预后判断。作为湿热证的客观生物标记,未来基于SIRI指数的胰腺癌分型诊治有望进一步锁定清热化湿治疗的有效人患者群,。未来以围绕湿热证为起点,进行胰腺癌病程全周期证候演变规律相关研究将助力中医基于证候的胰腺癌精准治疗。

     

    Abstract: The increasingly innovative cancer prevention and treatment strategies have achieved remarkable results in the prevention and treatment of various malignant tumors, while the rising trend of incidence and mortality of pancreatic cancer (PC) has not yet seen a significant turnaround. The clarification of the material basis of traditional Chinese medicine (TCM) syndromes can contribute to the multidisciplinary integrated diagnosis and treatment of PCpancreatic cancer. Long term clinical practice and research have confirmed that The damp-heat syndrome is the core syndrome of PCpancreatic cancer patients, .and have a crucial material basis of inflammatory microenvironment, accordingly, formula with the efficacy of Hheat-clearing and dampness-removing therapy can inhibit the malignant phenotypes by remodelling the tumor microenvironment. Conventional therapy plus with the heat-clearing and dampness-removing formula can make PC patients a better quality of life and significantly prolonged their survival time. can improve the quality of life of pancreatic cancer patients and prolong their survival time. Through the establishment of a standardized animal model of the damp-heat syndrome for basic research, it is found that inflammatory microenvironment is an important material basis of damp-heat syndrome. Clinical transformation research shows that The systemic inflammatory response index (SIRI) is intimately correlated with the cytokines changes underlying the damp-heat syndrome, and can be used to direct PC prognosis assessment. In the future, as an objective biomarker of the damp-heat syndrome. Thethe PC diagnosis and treatment of pancreatic cancer based on SIRI classification willfurther distinguishes the effective patients subgroup of patients responsive totreated with heat-clearing and dampness-removing therapy. In the future, tTaking damp-heat syndrome as the corestarting point to explore, the research on the whole period of syndrome evolution throughout the course of PCpancreatic cancer will contribute to syndrome-based TCM precision treatment of PCpancreatic cancer with TCM.
