胸腔内免疫治疗、化疗对恶性胸水免疫指标的影响

Effects of Intrathoracic DC-CIK Treatment and Chemotherapy on Immune Indexes in Malignant Pleural Effusion

  • 摘要: 目的 探讨胸腔内DC-CIK回输、腔内化疗对恶性胸水中淋巴细胞亚群及VEGF的影响及其机制。方法应用流式细胞技术检测经不同方法治疗前后胸水中淋巴细胞亚群比例的变化;应用ELISA法检测经不同方法治疗前后胸水中VEGF的变化。结果(1) DC-CIK治疗组,胸水中CD3-CD16+CD56+NK细胞、CD3+CD16+CD56+T细胞、CD8+CD28+T细胞较治疗前升高(P值均<0.05);上清液中VEGF浓度较治疗前有下降趋势(P>0.05)。 (2)化疗组,胸水中CD3+T细胞、CD3+CD4+T细胞、CD4+ CD25+T细胞较治疗前显著下降(P值均<0.05);上清液中VEGF浓度较治疗前显著下降(P<0.05)。结论胸腔内DC-CIK回输通过增强局部抗肿瘤免疫发挥作用;胸腔内化疗可通过改善局部免疫抑制状态及降低VEGF水平控制胸水。

     

    Abstract: Objective By comparing the differences of lymphocyte subsets and VEGF levels before and after DC-CIK treatment and intrathracic chemotherapy of MPE(malignant pleural effusion),we try to discover the possible therapeutic mechanism. Methods The changes of lymphocyte subsets and VEGF levels were detected by FCM and ELISA respectively before and after the different treatments. Results (1)DC-CIK group,the percentage of CD3-CD16+CD56+NK cells,CD3+CD16+CD56+T cells and CD8+CD28+T cells were significantly increased(P<0.05) and the level of VEGF was slightly decreased(P>0.05)in the fluids of post-treatment.(2) Chemotherapy group,the percentage of CD3+T cells,CD3+CD4+T cells and CD4+CD25+T cells as well as the level of VEGF are significantly decreased(P<0.05). Conclusion (1)The possible mechanism of DC-CIK transfusion is to enhance the local immune function;(2)Besides killing the tumor cell directly,the possible mechanisms of intrathoracic chemotherapy involves in improving the local immune suppression and lowering VEGF level.
